Community Nursing Service

The focus of our Community Palliative Care team is to make it possible for patients with specialist palliative care needs to be cared for at home where they feel most comfortable.

Our team maintains a close working relationship with other health professionals involved in your care.  We have a team of nurses who will visit you as frequently as you require, depending on your condition, in order to make your stay at home possible.

During these visits you are welcome to discuss any patient or family concerns.  These may go beyond your medical concerns.  They might be spiritual, social or practical concerns.  If our nurses can’t assist you, they will make sure you get the help you require through our Family Support Service or with Allied Health professionals.

Medical and nursing advice and support is available 24/7, which may be in person or via the phone.

Should you require specialist equipment we can arrange this for you too.
