Community Nursing Services
Our Community Palliative Care team make it possible for patients with specialist palliative care needs to be cared for at home where they feel most comfortable.
Our team maintains a close working relationship with other health professionals involved in your care. Our nurses will visit you as frequently as you require, depending on your condition, in order to make your stay at home possible.
Medical and nursing advice and support is available 24/7, which may be in person or via the phone. If you require specialist equipment we can arrange this for you too.
Our Community Palliative Care team offer a holistic approach to your care because we know that your wellbeing and the wellbeing of your family goes beyond your physical health. We are here to look after ‘the whole of you’.
