Meet the team

Te Omanga Residential Aged Care Team supports patients, their families, and staff working in rest homes and hospitals throughout the Hutt Valley.

Ros McIntosh and Mel Humphries are both clinical nurse specialists with experience in palliative care for elderly people living in residential aged care. Their aim is to increase the confidence and capability of Residential Care homes in the Hutt Valley to deliver palliative care for the older person.

The Aged Care Team works alongside nurses, caregivers and GPs in an advisory role, coaching and mentoring – acknowledging that palliative care work already takes place, and the needs of the frail elderly are varied and often complex.

As well as providing clinical advice the team is available to support families of the older person with palliative needs, and referrals to the service often come following discharge from hospital and transition into residential care.

The team also supports patients who have been transferred from the hospice’s In-patient unit to a facility for continuing care. Although normal daily care is performed by nurses and caregivers at the facilities, staff can access advice from nurses and doctors at the hospice at any time, and Ros and Mel provide regular support.
